We are looking for an individual seeking a career opportunity with one of the largest employers within the Tampa Bay area. + **Facility:** Sarasota Memorial HomeCare + **Location:** Sarasota, FL + **Status:** Full Time + **Shift Hours:** 8:00 AM - 5:00 PM / Monday - Friday + **Weekend Work:** Every Third Weekend + **On Call:** No + **Home visits in assigned service area:** Venice, FL **BayCare offers a competitive total reward package including:** + Benefits (Health, Dental, Vision) + Paid time off + Tuition reimbursement + Mileage reimbursement + 401k match and additional yearly contribution + Yearly performance appraisals and team award bonus + Community discounts and more The Home Health LPN provides hands-on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols. **Responsibilities include:** + Delivery of patient care to assigned patients, provides care for clients as ordered including IV infusions, infusion pumps, and tube feedings under the supervision of a Registered Nurse + Identifies, implements, and maintains appropriate teaching plans for patient and caregivers under direction of a Registered Nurse + Follows plan of care and collaborates with Registered Nurse regarding patient response to treatment and change in status + Utilizes nursing knowledge in the administration of medication and evaluation of effectiveness when applicable. **Required Certifications and Licensures:** + BLS (Basic Life Support) + LPN (Registered Nurse) License + Driver's License State of Florida (Florida driver's license is required within 30 days of hire) **Education:** + Required Associate's or Diploma Nursing **Experience:** + Required 1 year Licensed Practical Nurse + Preferred 1 year Home Care Why join BayCare HomeCare?
