Position Title: Home Health LPN Nurse - Park Ridge/Chicago, IL Area - Work from home with travel to patient homes and partner meetings.

Employment Status: Full-time (40 hours/week).

Location: Park Ridge/Chicago, IL. Hybrid position; Remote with occasional travel to client homes and home health partner meetings. Installs and clients are located in the Chicago area.

Position Summary: We are seeking a tech-savvy, LPN Nurse to ensure the safety and well-being of clients living independently at home by combining proactive wellness oversight with advanced technology. You will work closely with our alert center to monitor clients' wellness data, take action on critical alerts, and maintain regular virtual communication with clients and their families. You will also conduct wellness assessments during installation visits and develop personalized wellness plans tailored to each client's needs.

Flexibility, self-motivation, quick thinking, and strong communication skills are essential in this role as you navigate the evolving environment of a growing startup.

Key Responsibilities:

In-Person Responsibilities:

• Install and configure the Lifeguard Lookout system in client homes, ensuring proper placement of radar-based fall detection sensors and wellness monitoring devices.
• Conduct home environment assessments, identify potential safety risks, and document observations for the Lead RN.
• Educate clients and families on how the system works, addressing any questions or concerns.

Virtual Responsibilities:

• Monitor daily wellness data from the Lifeguard Lookout platform, tracking mobility patterns, falls, nighttime activity, and other health indicators.
• Prepare and submit weekly wellness reports summarizing key insights for the Lead RN’s final review and approval.
• Conduct virtual check-ins with clients to discuss wellness observations and provide recommendations.
• Escalate concerns regarding significant changes in behavior, potential health risks, or safety concerns to the Lead RN.

Collaboration & Support:

• Work closely with the Lead RN to provide detailed insights about each client’s health, home environment, and daily routines.
• Communicate regularly with caregivers, families, and care facilities, offering proactive wellness support.
• Troubleshoot basic technical issues related to system setup and escalate more complex issues to the Lifeguard Alert Center for resolution.

Required Education and Experience:

• Must have an Active LPN license in Illinois.
• Must reside and work in the Park Ridge/Chicago area.
• 2+ years of experience supporting senior populations, either in-home or remotely, focusing on wellness.
• Valid driver’s license and access to a car.
• Must have an active and working smartphone - used to download the installation app.
• Excellent written and verbal communication skills to effectively communicate with clients, families, partners and team members.
• Experience with wellness planning or coordination is a plus.
• Ability to thrive in a fast-paced, changing environment and adapt to new challenges.
• Strong proficiency in technology tools (Microsoft Outlook, Teams, Google Drive) and comfort with software applications.
• Open and curious about technology with a willingness to adopt and learn new tools to improve wellness oversight.
