Job Description The social worker plays a vital role in supporting patients and families by assessing their social, emotional, and financial needs. They collaborate with healthcare teams to develop and implement discharge plans, provide crisis intervention, and connect patients with community resources. Additional responsibilities include participating in care planning meetings, maintaining accurate documentation, and offering after-hours support on a rotating basis.
